Home From Hospital

Assisted Discharge Service

Based at North Manchester General Hospital, our team transport people home safely who have no other suitable means of getting home.

The aim of this service is to identify vulnerable or isolated patients prior to discharge and provide personalised support so they are prepared, transported and settled back into their home.

Independence Support Work

Our Independence Support Workers help people to settle back into their homes by providing tailored practical and emotional support, helping people to re-adjust to home life after a change in health and a hospital visit.

A combination of home-visiting and telephone support for up to 4 weeks to help people

Home from Hospital Adaptations

Our Handyperson team complete works to aid hospital discharge within 48 hours to help people safely return home from hospital.

We receive referrals directly from the hospital and can fit essential aids such as key safes, grab rails and bannister rails. We can also move furniture to make room for a hospital bed or other equipment, as well as securing any trip hazards in the home.

Telephone Support

Our team make calls to over 60’s recently discharged from, or who have recently attended A&E at North Manchester General Hospital, Manchester Royal Infirmary or Wythenshawe Hospital

Our phone support checks to see how the person is doing back at home and whether they need any support.

We can refer on to our own services if appropriate, as well as signpost on to other suitable services across the city.

Home First Project

This project provides a bespoke casework service for people about to return home after a stay on a mental health unit. The service includes:

A home assessment, looking at the condition of the clients property and home environment

A negotiated plan for any works needed to make the clients home fit for their return

Delivery of works as required and agreed.
